Notice of Conveyance/Change of Use

Document Overview

Submission Requirements

Form Sections

The document requires the following information to be filled out:

• Owner Details: Names and addresses for both Current Owner(s) and Proposed Owner(s).
• Contact Info: Phone number.
• Property Identification: Tax Parcel Number(s) and the original Date of Enrollment in Clean and Green.
• History: Confirmation if any land has been previously conveyed from the original tract.
• Enrollment Status: Selection of current enrollment type (Agricultural Use, Forest Reserve, Agricultural Reserve, or Ineligible Use).
• Conveyance Details:
  • Whether the entire parcel is being conveyed.
  • If splitting the parcel, the number of acres involved.
  • The proposed new use for the land (e. g. , Residential home site, Commercial, Forest Land).
• Usage Description: A description of how the land will be used and the number of acres affected.
• Signatures: Date of submission, proposed date of conveyance, and the signature of the current landowner.

• If the form is signed by someone other than the owner, authorized documentation must be provided.
