Document Overview
This is a form for the Pennsylvania Farmland and Forest Land Assessment Act of 1974 (Clean and Green). It is used to notify the Blair County Assessment Office of any change in land use, ownership transfer, or division of property currently enrolled in the program.
Submission Requirements

Form Sections
The document requires the following information to be filled out:
- Owner Details: Names and addresses for both Current Owner(s) and Proposed Owner(s).
- Contact Info: Phone number.
- Property Identification: Tax Parcel Number(s) and the original Date of Enrollment in Clean and Green.
- History: Confirmation if any land has been previously conveyed from the original tract.
- Enrollment Status: Selection of current enrollment type (Agricultural Use, Forest Reserve, Agricultural Reserve, or Ineligible Use).
- Conveyance Details:
- Whether the entire parcel is being conveyed.
- If splitting the parcel, the number of acres involved.
- The proposed new use for the land (e. g. , Residential home site, Commercial, Forest Land).
- Usage Description: A description of how the land will be used and the number of acres affected.
- Signatures: Date of submission, proposed date of conveyance, and the signature of the current landowner.
Important Notes
- If the form is signed by someone other than the owner, authorized documentation must be provided.
