Submission Requirements
The form requires signatures from two parties to certify the information:
- Funeral Director / Cemetery Board: Certifies the date of burial and selects the payment recipient.
- Director, Blair County Veterans Affairs: Certifies that the Military Discharge and Death Certificate have been examined and are true.
Mandatory Attachments: Copies of the Death Certificate and the veteran’s Wartime, Honorable, Active Duty Discharge must be provided with this application.
