  • Veteran’s Name, Service #, Date of Birth, and Date of Death.
  • Legal residence at the time of death.
  • Specific War served, Dates of Enlistment and Discharge, Branch of Service, Last Unit Assigned, and Rank.
  • Cemetery location and Next of Kin details (Name, Relationship, Address).

Submission Requirements

The form requires signatures from two parties to certify the information:

  1. Funeral Director / Cemetery Board: Certifies the date of burial and selects the payment recipient.
  2. Director, Blair County Veterans Affairs: Certifies that the Military Discharge and Death Certificate have been examined and are true.

Mandatory Attachments: Copies of the Death Certificate and the veteran’s Wartime, Honorable, Active Duty Discharge must be provided with this application.
