Application Overview

- Residency: Legal residence at the time of death.
Certification and Signatures
The form requires three distinct sections to be completed and signed:
Next of Kin:
- Must provide their name, relationship to the veteran, and address.
- Must sign a certification of truth.
Funeral Director:
- Must certify the date the veteran was buried.
- Must indicate who receives the payment (Funeral Director or Next of Kin).
- Must provide their address, phone number, and signature.
Director, Blair County Veterans Affairs:
- Official certification that the Military Discharge and Death Certificates have been examined and found true.
- Must sign to confirm the veteran meets all residency and service requirements.
Submission Requirements
To complete the application, copies of the following documents must be provided:
- The Death Certificate.
- The Veteran’s Wartime, Honorable, Active Duty Discharge papers.
