Overview
This is a two-page application form for the City of Altoona regarding floodplain elevation and construction permits. It is designed to be filled out by the applicant on Page 1 and completed by the Floodplain Administrator on Page 2.
Application Requirements (Page 1)
Applicants must provide the following information:
- Site Details: Location, lot size, and owner/applicant contact information.
- Project Scope: Type of construction (e. g. , New Construction, Alteration), usage (Residential, Commercial, etc. ), and construction type (Frame, Masonry, etc. ).
- Financials: Cost of the project and the Building Permit Fee.

- Building Permit
- Site Plan (showing floodplain limits)
- Building Plan (showing elevations)
Fees:
- Application Fee: $100 (due with application).
- Partial Building Permit: Required upon approval; cost is the greater of $1,500 or 50% of the Building Permit Fee.
General Provisions
The applicant must agree to the following conditions:
- All work must conform to the City of Altoona Floodplain Ordinance.
- No work may begin until the permit is issued.
- The permit may be revoked for false statements; if revoked, all work must cease until reissued.
- The development cannot be occupied until a Certificate of Compliance is issued.
- The permit expires if work does not commence within six months.
- Other local, state, or federal permits may still be required.
- The applicant consents to inspections by the Local Administrator.
Administrator Review (Page 2)
The Floodplain Administrator uses this section to:
- Determine if the site is inside or outside the 1% Annual Chance Area (flood zone).
- Identify specific waterways (e. g. , Brush Run, Mill Run) or floodway status.
- Record technical data such as Panel Number, Base Flood Elevation, and Design Elevation.
- Approve or deny the application (issuing a Location and Elevation Permit if approved).
- Record inspection dates and notes.
