- General Info: Year built, year of most recent renovation, total number of rooms (excluding hallways/bathrooms), and number of bedrooms.
- Exterior & Systems: Roof cover, exterior siding, windows, electrical, heating unit, and central A/C.
- Bathrooms: Count of full and half bathrooms.
- Basement: Percentage of finished basement, type (slab, crawl space, piers), and square footage of finished lower levels.
- Heating & Cooling: Heat distribution type (e. g. , forced air, steam), heat source (e. g. , coal, electric, gas), and number of fireplaces.
- Utilities: Sewer connection type (e. g. , public, septic) and water connection type (e. g. , public, well).
- Other Features: Permanent staircase to attic, finished attic status, deeded acreage, and purchase price within the last three years.
- Market Value: A section to list factors that might affect the potential market value of the property.

Submission
The completed form must be mailed using the provided envelope to the Reassessment Office. The bottom of the form requires a signature from the owner, tenant, or other authorized person, along with a daytime telephone number.
