To complete the application, the following information must be provided:
Owner Information:
- Name
- Birthdate (required if over 65 years old)
- Address, City, State, and Zip Code
- Phone Number (Primary and Alternative)
Dog Information:
- Dog's Name
- Breed
- Age
- Color
Important Notes and Restrictions
- Vendor Fees: If applying through a vendor, an additional 50 cents per license will be charged.
- Senior/Disabled Proof: Applicants claiming senior or disabled status must show proof of eligibility.
- Lifetime License Location: Lifetime licenses may only be obtained at City Hall.
