Required Information

To complete the application, the following information must be provided:

Owner Information:

• Name
• Birthdate (required if over 65 years old)
• Address, City, State, and Zip Code
• Phone Number (Primary and Alternative)

Dog Information:

• Dog's Name
• Breed
• Age
• Color

Important Notes and Restrictions

• Vendor Fees: If applying through a vendor, an additional 50 cents per license will be charged.
• Senior/Disabled Proof: Applicants claiming senior or disabled status must show proof of eligibility.
• Lifetime License Location: Lifetime licenses may only be obtained at City Hall.
