- Selected Characteristics: Fields for the principle type of frame (e. g. , Masonry, Wood), sewage disposal, water supply, dimensions (stories, square footage), and heating fuel.
- Demolition Work Procedures: A numbered list of mandatory rules for demolition:
- Right-of-Way: Encroachment into public right-of-way requires approval from the Department of Public Works.
- Site Plan: A site plan showing adjacent streets and infrastructure must be submitted.
- Safety & Access: Public rights-of-way must remain free of debris. Equipment on tracks requires street protection.
- Financial Security: A deposit may be required to protect city-owned improvements.
- Notification: The Department of Public Works must be notified 24 hours prior to starting demolition and before the sewer cap inspection.

Sewer Cap Inspection Procedures (Page 4)

- Location: Caps must be outside the foundation wall unless an obstruction (like a sidewalk) prevents it.
- Inspection: The pipe must be inspected and the cap made in the presence of an Engineering Inspector.
- Access: Contractors must provide safe access; inspectors can refuse inspection if access is inadequate.
- Scheduling: The Public Works Department must be called at 814-949-2446 to schedule the inspection. A 24-hour advanced notice is required.
