This document is a Contractor's Application Packet for the Altoona Redevelopment Authority Land Bank. Application Requirements
Applicants must provide the following information and documentation:
- Firm Details: Name, business structure (Sole Proprietorship, Partnership, Corporation, or LLC), and supporting documentation listing owners and authorized signatories.
- Contact Information: Physical address, Federal Tax ID or Social Security Number, telephone numbers (office and cell), fax number, and web address.
- Business Certifications: Indication of status as a Minority Business Enterprise, Women’s Business Enterprise, Disadvantaged Business Enterprise, VOSBE (Veteran Owned Small Business), or SBE (Small Business Enterprise). Certification must be provided if "Yes" is selected.
- Demographics: A table to report the number of employees by race (White, Black/African American, Asian, etc. ) and Hispanic origin.
- Operational History: Date the firm was organized, number of full-time and part-time employees, and a list of sub-contractors used for plumbing, electrical, and mechanical work.
- Licensing: Copies of the Bureau of Consumer Protection Registration (with registration number) and the City’s Business Privilege License.
- Contract History: Disclosure of any past defaults or failures to complete contracts, with an explanation if applicable.
- Project Portfolio: A list of three major projects recently completed in the Altoona or Blair County area, including approximate cost, date of completion, address, and a contact person.
Equal Employment Opportunity (EEO) Certification
The packet includes an Attachment II requiring the contractor to certify that they are an Equal Opportunity Employer. Key points include:
- Non-Discrimination Policy: The firm agrees not to discriminate against employees or applicants based on race, color, religion, sex, national origin, disability, age, gender identity, genetic information, sexual orientation, or military status.
- Scope: This applies to recruitment, compensation, training selection, and promotion/termination decisions.
- Consequences of Non-Compliance: Failure to comply may result in contract cancellation, termination, or suspension. The company may also be removed from the Approved Contractors List.
- Subcontracting: The EEO provisions must be included in every subcontract, sublease, or purchase order related to City of Altoona contracts.
