Overview

Application Requirements

To complete this application, the following information must be provided:

  • Personal Details: Names of the widow and veteran, legal residence at death, dates of birth and death for both parties.
  • Burial Information: Cemetery location and the veteran's date of death.
  • Military Service Record: Branch of service, dates of entry and discharge, war served in, rank, and unit last assigned.
  • Next of Kin: Contact information (name, relationship, address) for the widow's next of kin.

Financial Allowance

  • Amount: A burial allowance of $75. 00 is available.
  • Payment: The funds shall be paid to the Funeral Director or Next of Kin as indicated on the form.

Required Signatures and Certifications

The form requires three distinct sections to be completed:

  1. Next of Kin Certification: The next of kin must certify the truth of the information provided.
  2. Funeral Director Certification: The funeral director must certify that the widow was buried and sign off on the payment recipient.
  3. Director of Veterans Affairs Certification: The Director must verify that:
    • Military Discharge and Death Certificates are provided.
    • The veteran was honorably discharged from active duty during wartime (other than training purposes).
    • The widow was a legal resident of Blair County at the time of death.

Submission Instructions

  • Required Documents: Copies of the widow’s death certificate and the veteran’s wartime, honorable, active duty discharge must be attached.
  • Non-Blair County Burials: If the veteran was not buried in Blair County, a copy of the veteran’s death certificate must also be provided.
  • Version Control: This version was revised in December 2013; all other versions are obsolete.
