Burial Allowance Application - Wartime Veteran (Honorable)

Overview

• Payment Recipient: The payment may be made to either the Funeral Director or the Next of Kin.

Required Information

Applicants must provide detailed personal and military history, including:

• Veteran Details: Name, Service Number, Date of Birth, Date of Death, and Legal Residence at death.
• Military Service: War served in, Date of Enlistment, Date of Discharge, Branch of Service, Last Unit Assigned, and Rank.
• Burial Information: Cemetery and Location.
• Next of Kin: Name, Relationship to the veteran, and Address.

Required Signatures

The application requires certification from three parties:

1. Next of Kin: Must sign a certification of truth regarding the information provided.
2. Funeral Director: Must certify the date the veteran was buried and sign off on the payment recipient.
3. Director, Blair County Veterans Affairs: Must examine and certify that the Military Discharge and Death Certificates are true and that the veteran meets all eligibility requirements.
